Medical Certificate for visa application
Are you planning an extended stay in a foreign country for work or study? Each country has different requirements for your state of health. A Certificate of Health, determined through a medical certificate could be a requirement for the application process.
Keurdokter can perform these medical examinations for students, expats and particularly people going abroad who plan to stay for a longer period of time. Once these forms are in order you will be ready to embark on your journey knowing you have all the correct and accurate documents in place.
1. Standard Health Declaration
Keurdokter is qualified to perform your medical certificate for visa application. These examinations are to be performed by a doctor other than your regular (family) doctor. If you do not have a specific form from the embassy or university, then we can offer you the standard Health Certificate. Keurdokter will perform a complete body medical examination.
The medical examination involves:
- Blood pressure
- Eyes Measurement
- Uranalysis
- Length/weight/BMI
- Physical examination
- Blood analysis: cholesterol, HDL/LDL/Triglyceriden.
Once all the results are gathered and everything is in order you will receive your Certificate of Health.
The health certificate can be provide in English, Dutch, French or Polish.
2. You received a medical report from the embassy
The requirements per country can be different. This may involve, for example, an extra physical examination, a chest X-ray or a blood sample that is tested for HIV or hepatitis. This is stated on the form that you receive when applying for the visa.
As soon as you make an appointment with us, you will receive a summary of what to bring with you. You bring the form from the embassy or university with you. We know how to approach the right people within the embassy for specific questions. This way you will leave well prepared and with a peace of mind to your destination.
